We check the shape of the pores, since round pores usually mean oil and oval pores usually mean lost collagen. We run a hand over the skin, because roughness you can feel is a different problem from marks you can only see. Where there are dips rather than dots, we will tell you that this is really acne scarring and route you accordingly. Redness like mild sunburn for one to three days after needling, then a slightly sandpapery feel as the skin renews. No makeup for twenty four hours and daily sunscreen throughout. Collagen builds over three to six months.
Home care manages congestion well and manages collagen poorly, which is a useful way to think about the division of labour.
A salicylic acid cleanser or serum is genuinely effective, because salicylic acid is oil soluble and gets into the pore lining rather than sitting on top of it. Niacinamide helps regulate oil production over time. A retinoid improves turnover, keeps openings clear and, used for a year or more, modestly improves the collagen around them. Daily sunscreen protects the scaffolding you still have. Pore strips remove the top of a blackhead and nothing else, and squeezing stretches the opening permanently, so both are best avoided.
Where home care stops is with pore size itself. Nothing applied topically rebuilds the collagen ring holding a stretched pore open, so if your pores have changed shape rather than simply filled up, no routine will refine them. Clay masks and cold water tighten the surrounding skin for an hour or two, which is real but very temporary.
