A jawline softens for three reasons at once

Almost everybody calls this sagging, and sagging is only a third of the story. Which third dominates yours is what decides whether a treatment sharpens the line or barely touches it.

The first is loss of support from above. The cheek fat pads that once sat high on the face shrink and slide downward, and the bone beneath them quietly resorbs. What used to be held up is no longer held up, so it gathers along the jaw. This is why restoring cheek volume can sharpen a jawline without anything being done to the jaw itself, and why treating the cheek above a jowl so often does more than treating the jowl itself.

The second is genuine skin laxity. Collagen and elastin decline from the mid thirties, so the skin envelope loses its recoil and no longer sits tightly over the structure underneath. Pinch the skin at your jaw and let go: if it takes a moment to settle back, laxity is a real part of your picture and tightening will help.

The third is fat that has accumulated under the chin, which blunts the angle between neck and jaw regardless of how good the skin is. That is a volume problem in the opposite direction from the first one, and it needs reducing rather than restoring.

Most people over forty have some of all three, and non surgical treatment improves definition meaningfully in the great majority of them. Where there is a great deal of loose skin, a surgical opinion gives the better result, and you will hear that at the assessment.

Three of these are doctor led and CACI is therapist led. Where the real cause is loss of cheek support rather than laxity, we will send you to facial volume loss instead, because restoring the structure above a jowl often does more than treating the jowl.

How Dr Mattia assesses a jawline

The assessment is done straight on and in profile, and it involves a good deal of pinching, because the single most useful question is whether the skin still recoils.

Dr Mattia Parducci checks the projection of your cheekbones, since a flat mid face often explains a soft jaw entirely. He assesses skin recoil at the jaw and neck, looks at the angle between chin and neck in profile, and establishes whether there is a genuine fat pocket underneath. He will ask about recent weight change, which alters both the picture and what is realistic.

Where there is a great deal of loose skin, he will tell you that non surgical treatment will improve it but not transform it, and that a surgical opinion may serve you better. That conversation happens at the assessment rather than after you have paid for a course.

Before your appointment

Bring a photograph from a few years ago if you have one. Avoid alcohol and anti-inflammatory painkillers for two days before Morpheus8 or Endolift. Tell us about recent or planned weight change.

During the appointment

HIFU takes around forty five minutes and needs no numbing, though it is felt as deep warmth. Morpheus8 uses numbing cream and takes around forty five minutes. Endolift is performed under local anaesthetic and takes about an hour.

Afterwards

HIFU and CACI have no downtime at all. Morpheus8 leaves redness for two to three days. Endolift involves swelling and bruising for several days. All of the collagen based results build over three to six months, so we review at three months.

Protecting your jawline at home

Less than for most concerns on this site, and it is worth being straight about that.

What genuinely helps: daily sunscreen, because ultraviolet exposure degrades the collagen and elastin holding the jawline in place faster than time alone. A retinoid used consistently thickens the skin over months, which improves how it drapes. Keeping your weight stable matters, since repeated loss and regain stretches the envelope. Sleeping on your back rather than face down reduces the compression that deepens folds over decades. Good posture and not smoking both make a measurable difference over years.

What will not work: facial exercises and jawline devices, which are widely promoted and have no good evidence behind them. The muscles are not the reason a jawline softens, and in the mid face more movement can accelerate fat pad depletion. Creams marketed as lifting tighten the surface for an hour or two at most; nothing applied topically reaches the depth where laxity is created.

Can jowls be treated without surgery?

Mild to moderate jowls, yes, and often to a degree people are pleased with. Where there is a great deal of loose skin, non surgical treatment improves rather than corrects it, and we will tell you if that is your situation.

Which is better, HIFU or Morpheus8?

HIFU reaches deeper and needs no needles or downtime. Morpheus8 treats skin quality and texture at the same time as tightening. If your skin is crepey as well as lax, Morpheus8 usually gives the better overall result.

Does CACI jowl lift actually work?

For early softening and for maintaining a result, yes, and it is pleasant and needs no recovery. It will not correct established laxity. Taken as a course of ten with monthly upkeep, it does what it claims.

How long do results last?

HIFU is usually repeated annually. Morpheus8 results last one to two years. Endolift is generally longer lasting again. None of them stops the ageing that caused the change in the first place.

Will filler help my jowls?

Often, but not by filling the jowl. Restoring cheek support above it lifts the whole area, and a small amount along the jaw can sharpen the line. Injecting into the jowl itself usually makes it heavier.

Can you treat a double chin?

Yes. Where it is a fat pocket rather than loose skin, Endolift reduces it and tightens in the same treatment. That combination is why it suits this area particularly well.

How soon will I see a difference?

Not immediately, with any of the collagen based treatments. Expect the first change at around six weeks and the full result between three and six months. Anything promising a same day lift is describing swelling.

Does HIFU hurt?

It is felt as deep heat in short bursts, more intense over bone. Most people manage comfortably without numbing, and it is over in around forty five minutes with nothing to recover from afterwards.

I lost weight and my jaw looks worse. Why?

Fat left the face but the skin did not retract, and you also lost the cheek support that was holding the jawline up. It is common and frustrating. Treatment usually needs to address volume and laxity together.

Am I too young or too old?

Most people who benefit are between their early forties and mid sixties. Earlier than that there is often nothing to treat, and beyond a certain degree of laxity a surgical opinion gives a better outcome.

Do I need a consultation first?

Yes. Jawlines are the clearest example of a concern where treating the visible problem rather than its cause falls short, and the cause is not something a photograph will show.
