Almost nobody arrives with one problem. Eye bags is a single phrase covering at least five different things, and they respond to completely different treatments. So the useful question is not which treatment you want, but what is actually casting the shadow.
Sometimes the skin itself is the issue. Under-eye skin is the thinnest on the body, so as it loses collagen it turns crepey and translucent, and the blue of the vessels underneath begins to show through. Sometimes the problem is volume. The cheek falls away slightly, a groove opens up between the lower lid and the cheek, and the light that used to land there now falls into a shadow. Sometimes it is fluid, which is why the same face looks different at breakfast and at lunchtime. And sometimes it is the fat pad that cushions the eye pushing forward through a weakening membrane, which is a structural change no injectable will fix.
Pigment, allergy, thyroid problems and genetics all play a part too, and plenty of people have two or three of these at once. The NHS guidance on bags under the eyes is a sensible starting point if you want to rule out a medical cause first.
Where treatment fits the pattern, it usually helps a great deal. Where the answer is a surgical referral or simply better sleep, you will hear that at the consultation and leave knowing exactly where you stand.
Have a look through these before you read about treatments. Most people recognise themselves in one or two, and that is what decides the route.
Colour, not volume
The skin is flat but darker, and concealer never quite covers it. This is usually thin skin letting the vessels underneath show through, sometimes with pigment on top. Skin quality treatments help here, filler usually does not.
Volume
You can feel a distinct dip between the lower lid and the top of the cheek, and it is deeper when you are tired. Light falls into the groove and reads as a bag. This is the pattern that responds best to carefully placed under-eye filler.
Skin quality
Fine lines that stay put when your face is still, and skin that looks like tissue paper when you smile. This is collagen loss in the thinnest skin you have. It responds to treatments that rebuild the skin rather than fill underneath it.
Fluid
Worse in the morning, worse after salt, wine or a bad night, better by the afternoon. That daily swing tells you it is fluid rather than structure. Gentle lymphatic and toning work can help, and so can some unglamorous changes at home.

Under-eye dermal filler
There is no single best under-eye treatment, because the four patterns above are four different problems. This is what each route is genuinely good at, and what it costs.

Three of these are carried out by Dr Mattia Parducci, our GMC registered doctor, and one by our aesthetic therapists. Which one suits you depends entirely on the pattern above, and matching it properly beats defaulting to filler. Skin health work such as HydraFacial and Celluma light therapy often runs alongside these.
Injectable, doctor led
A soft hyaluronic acid gel placed deep against the bone to rebuild the ledge the cheek has lost, so light stops falling into the groove. Placed conservatively and reviewed at two to four weeks. It is dissolvable, which is a genuine safety net in an area this delicate.
Regenerative, doctor led
Purified DNA fragments that ask the skin to repair itself rather than filling anything. Over a short course the under-eye skin thickens a little and the bluish translucency softens. This is our first choice when the problem is skin quality rather than volume.
Microneedling with radiofrequency
Fine needles deliver radiofrequency energy below the surface, which tightens and rebuilds collagen where the skin has gone crepey. The eye handpiece is designed for exactly this area. Expect redness for a couple of days and a result that keeps improving for months.
Non invasive, therapist led
Microcurrent and lymphatic work that lifts and de puffs without needles of any kind. It will not fill a hollow, and we will not pretend it does, but for fluid, tone and tired looking eyes it is a genuinely pleasant and effective option.
The assessment matters more here than almost anywhere else on the face, because the same complaint has four different causes and the wrong treatment can make things look worse rather than better.
Dr Mattia Parducci looks at where the shadow actually falls, whether it changes when you tilt your head or lie back, how much you can pinch the skin, how firm the pad is, and whether the swelling varies through the day. He will ask about sleep, allergies, thyroid problems and family history, because plenty of under-eye shadow is inherited and was never going to respond to treatment.
If you would be better served by an oculoplastic surgeon, or if nothing needs doing at all, he will tell you. Under-eye filler in particular is a treatment we decline more often than we perform, because placing it into skin that is puffy or into a pad that is already full is how people end up looking worse.
Come without eye makeup if you can, and bring a photograph of yourself from a few years ago if you have one. Avoid alcohol and salty food the night before, so we see your baseline rather than a fluid day. Tell us about any blood thinning medication or supplements.
We assess first and treat second, and sometimes we do not treat on the same day. Injectable treatments use a blunt cannula where appropriate to reduce bruising, and take around twenty to thirty minutes. Morpheus8 and CACI take longer and use numbing cream beforehand.
Expect some swelling for a few days with anything injected, and possible bruising that makeup covers by the end of the week. Sleep slightly propped up for two nights. We review injectable results at two to four weeks, once everything has settled, before deciding whether anything more is needed.
A fair amount of under-eye appearance is genuinely within your control, and it is worth trying the free things first.
Sleeping propped up slightly stops fluid pooling overnight. Cutting evening salt and alcohol makes a visible difference to morning puffiness. A cold compress for a few minutes helps for the same reason. Daily sunscreen around the eyes slows the collagen loss that causes crepe in the first place, and treating hay fever properly stops the rubbing that darkens the skin. A caffeine eye cream will genuinely reduce mild puffiness for a few hours, and a well formulated retinoid used carefully can thicken the skin over several months.
What home care will not do is rebuild lost volume, tighten skin that has already gone lax, or move a herniated fat pad. No cream reaches deep enough for any of that, whatever the packaging says. If your under-eye change is structural, creams will only ever be maintenance around the edges of it.

It depends entirely on what is causing them. Shadows, hollows and crepey skin all respond well to non surgical treatment. A true herniated fat pad does not, and only surgery will correct that. You will know which one you have at your assessment.

Less than most people expect. We use numbing cream and the filler itself contains a local anaesthetic. Where the anatomy allows, Dr Mattia uses a blunt cannula rather than a needle, which is more comfortable and reduces bruising.
Typically nine to eighteen months, though the under-eye area often holds results longer than other parts of the face because there is so little movement there. We review at two to four weeks and top up only if it is genuinely needed.
It can, if it is placed into the wrong pattern. Filling under an eye that is puffy with fluid, or under a fat pad that is already full, makes the area look heavier. This is exactly why we assess before we treat, and why we decline this treatment fairly often.
Usually one of three things, or a combination. Thin skin showing the vessels beneath, a hollow casting a shadow, or genuine pigment in the skin. Tiredness, allergies, dehydration and genetics all make each of them more obvious.
They are better for a different problem. Polynucleotides improve the quality of thin, crepey, shadowed skin. Filler replaces lost volume. If your issue is a hollow, polynucleotides will not fill it, and if your issue is skin quality, filler will not fix it.

Please do if the swelling came on suddenly, affects one eye only, is painful or red, or comes with vision changes. Persistent puffiness alongside tiredness, weight change or feeling cold is worth a thyroid check before any aesthetic treatment.
